Parents must complete the registration form below. All information must be provided in order for a student to be considered.

Club Description:

he GSA (Gender and Sexualities Alliance) is an after school club that welcomes all students and is designed to create a safe space for the LGBQT community and their allies.  The club strives to help students develop positive relationships between peers and work toward creating a more inclusive environment at Forsythe.

Does your student have a health condition requiring  emergency medications to be administered such as Diabetes, anaphylaxis to food allergies or insect stings, asthma, seizure emergency medications?

Please be informed that there is no access to emergency medications that have been provided to the school for use during the school day.  After hour programs require a separate set of these medications.  

Also note that it is the parent's role to reach out to the coach to inform them of their student's condition, provide appropriate training, and education to manage the individual aspects of their student.  If you need assistance with identifying that individual's contact information please reach out to Delia Powell at  powelld@aaps.k12.mi.us. 

Also please reach out to the school nurse to inform them of your student participation so that they can be a resource for any specific planning necessary for your student.

By signing this form I give permission for my child (named above) to participate in the Forsythe Middle School afterschool clubs.  I understand that my child is expected to adhere to all policies of their club leader, Forsythe Middle School, and Ann Arbor Public Schools.  I understand that I am to provide transportation for my child promptly at 4:30 p.m. on club days unless my child rides the late bus on Mondays, Tuesdays, or Thursdays.  We understand space is limited and selection is based on a first-come process. We also understand that students can be removed from the club at any time due to behavioral or attendance issues. (Parents, please type in your name if you agree with this statement) *
