Gateway School-Based Health Center: SURVEY FOR PARENTS or GUARDIANS
The purpose of this survey is to understand the health needs of the students attending Gateway High School and its feeder schools. You are being asked to take this survey because you are a parent or guardian of a child attending Gateway High School or one of its feeder schools.
Your response will help determine the needs of students and if there is a community commitment for a school-based health center. The school-based health center would provide health services for students attending Gateway or one of its feeder schools, or any younger children who are not yet in the school system but have siblings who attend these schools.
A school-based health center is a place where students can receive various health services. These services could include:
• Primary medical care (from a pediatric medical practice)
• Sports physicals
• Guidance for a healthy lifestyle
• Mental illness assessment and treatment
• Substance abuse assessment and treatment
• Dental services
• Reproductive/sexual health support.
The presence of a school based health center on the premises of Gateway High School would not change the role of the school nurse. The school nurse is a vital team member and provides supports for students utilizing clinic services.
Your input and feedback will support the design and services of the Gateway School-Based Health Center. This survey is 19 questions long and should take no more than 10 minutes of your time. Your response is confidential and will only be reported in summary form, so you cannot be identified individually.
