Chelsea Athletics Transportation Form
When the Chelsea School District and the Athletic Department provide transportation we believe it is in the best interest of the individual participants, as well as for team spirit and camaraderie for team members to travel to and from away contests in school-provided transportation.  However, in cases where the parents feel it necessary to NOT use school transportation, this form must be completed.  The form is good for only the date listed below.  A separate form will need to be filled out should a student need to ride home on another date.  Any other special requests from a parent will be granted on a case-by-case basis and should be given to the coach at least 24 hours in advance of the trip.  

In cases where the school district does NOT provide transportation it is the responsibility of the parent to provide, or arrange, transportation for the student to and from the contest.  This form is not required in those cases.  In that case, it is the sole responsibility of the parent to arrange transportation for their student.
