The Questionnaire has been co-produced by the peer led project MH:2K Nottinghamshire and the CAMHS involvement group. We are working together with Nottinghamshire Health Care Trust who are committed to the way they listen, involve and communicate with children and young people.

The NHS is a large organisation and this means that we don't get things right for everyone. We want your support to enable us to understand and make changes to get it right for children and young people.
This questionnaire is going to ask you questions around communication, involvement and feedback. This questionnaire only takes 10 minutes to complete but will have a huge impact on how communication, involvement and feedback can be improved.

In regards to confidentiality and anonymity we do not ask for names or details about you in this questionnaire.
