Yarra Warra 3 Year Old Pre-School Application Form
Enrolments for 3 Year Old Kindergarten at Yarra Warra Pre-School are accepted from the month children turn two years old.

Applications are completed by filling in the following online form and submitting a $20 registration fee. Alternatively, you can download a copy of the application form from the Yarra Warra Pre-School website. http://yarrawarrapreschool.org/3yoenrolments.

Applications for 4 Year Old Kindergarten need to be registered directly with Nillumbik Shire Council. Enrolments are accepted from the first business day in March, two years before your child is eligible to attend. To maximize your chance of getting your first preference, it is important to register at the beginning of March. No Jab, No Play Legislation
From 1 January 2016, all parents/guardians seeking to finalise enrolment of their child at an early childhood service
(including preschool) in Victoria must provide evidence that their child is:

• fully immunised for their age OR
• on a vaccination catch-up program OR
• unable to be fully immunised for medical reasons

‘Conscientious objection’ is not an exemption under the ‘No Jab No Play’ Legislation.

You can get copy of your child’s most recent Immunisation History Statement from the Australian Childhood Immunisation Register (ACIR) as evidence of your child’s immunisation.

If your child is not fully immunised, or if you are not sure if your child is fully immunised, or you’re interested to find out

• locate and consult your nearest immunisation provider
• review the ‘Immunisation – childhood’ fact sheet to find out what vaccines are due and when

Alternatively you can talk to your regular family doctor/GP for more information or to determine if your child is unable to be fully immunised for medical reasons. The GP will need to complete and sign a Medicare Immunisation Exemption Medical Contraindication Form, and send it to ACIR. The parent/guardian will need to obtain an updated Immunisation History Statement from ACIR and provide it to the preschool to finalise enrolment.

Parents/guardians offered a place will be asked to provide immunisation documentation upon acceptance of place that shows that their child's immunisations are up to date for their age or that an exemption applies.

I have read the above information regarding the "No Jab, No Play Legislation" and will be able to provide a copy of my child's Immunisation History at the time of enrolment.
Registration Fee Payment
A non-refundable registration fee of $20 is required as part of the application process to Yarra Warra Pre-School.

A direct deposit of $20 can be paid directly into the pre-school's account.
