Technology Repair Request
If you have a school issued Chromebook (Dell models only) or iPad that is in need of repair or not functioning properly, please return it to the school so that it can be sent out for repair prior to the end of the school year. Repairs are only available on our Dell Model Chromebooks and iPads.
WE ARE ASKING YOU TO RETURN THESE DEVICES NOW TO AVOID ANY REPAIR COSTS. ANY DEVICES RETURNED AFTER MAY 31ST MAY INCUR A DAMAGE/REPAIR FEE
Please note, we DO NOT have replacement devices at this time.
If you plan to return your device for repair, please complete the following form and then return your device to the Middle School location (1601 S Dearborn) within 24 hours. Please include your child’s name/grade on a post-it or label when returning the device.

Will not turn on/charge
Screen is damaged
Keys are damaged
Camera or microphone does not work
Additional details that will be useful when repairing the device (i.e. the S key does not work, there was accidental drop or spillage, etc.)
Never submit passwords through Google Forms.
