We realize that financial issues can arise unexpectedly and account deadlines may need to be altered.  

If a student needs to follow an alternative payment plan than the one listed above, they must complete this form for approval prior to the next payment deadline to FC Choirs, Inc.

If a payment deadline passes and an Alternative Payment Plan Agreement has not been submitted, approved, and followed, the student will not be allowed to participate in upcoming events.
