Athlete Registration Information
Please complete the form below to register your athlete for evaluations for our 2022-2023 season. Evaluations are a way for our program to properly set team levels and place your athlete on an appropriate team where they will not only feel secure and success, but also feel challenged and are able to grow.   Evaluations Day: June 26th, Makeups available by appointment for an additional fee.
Ages 4 & 5 (please contact the gym for team guidelines)
SESSION #1: Ages 6-12 (Athletes must be this age as of June 1, 2023)--> 2:30-4:00PM
SESSION #2: Ages 13-18 (Athletes must be this age  as of June 1, 2023)--> 6:30-8:00PM
Can’t make our evaluation day?  Please contact the gym to set up a private evaluation
*Athletes should attend the times for their age as of JUNE 1, 2023.  For example, if an athlete turns 13 on June 2, 2023, they should still attend session 1.  Your athlete may be asked to attend the second session depending on skill set.

$50 due (THIS COST IF FOR NEW GYM MEMBERS)
$25 due (CURRENT TEAM MEMBERS)

Double Team Option:  Please indicate below whether or not your athlete would like to be considered for more than one team in our program.  Please understand that this commitment means that the athlete will be required to attend all regular practices for both teams they are slotted for.  There is no additional monthly tuition cost, however, there will be additional cross over fees for competition registrations.  This option is for athletes who would like to participate on more than one team.  Athletes must be able to perform all necessary skills for both teams in order to secure a position on more than one North Fork Cheer competitive team.  This is not an option for every athlete and athletes may not be slotted for more than one team even if this is selected. *
