REQUEST FOR INITIATION OF ENHANCED DBS CHECK PROCEDURE

FEES:
Checks for volunteers are free of charge for Ross CDT members, Talk Community Hubs and local Good Neighbour Schemes.

For paid staff, an application for an enhanced DBS certificate costs £65 and is payable to Ross CDT (the Disclosure and Barring Service charge £49.50 and a £15.50 administration fee is payable to Ross CDT).  An invoice will be issued by the Ross CDT DBS checking team on receipt of this form.

ELIGIBILITY FOR ENHANCED DBS CHECKS:
We can only legally request Enhanced DBS checks if the applicant's role is legally classed as either 'Work with adults' or 'Work with children' or both.  To request an additional barred list check the applicant's role must also involve 'regulated activity'. 

Before completing the form (Question 9) please refer to the following guidance to decide which type of check the role is eligible for: https://www.gov.uk/find-out-dbs-check/y  If the role is not eligible (for example if working infrequently with children or vulnerable adults) we will not be able to proceed with the application.  In this case you can ask the employee to request a Standard DBS check.
