❗️IMPORTANT: This form is NOT for use by school administrators who are partnering with the Office of Equity on an investigation involving students.

If you believe that you have experienced or witnessed a bias-based or sexual misconduct incident in the Boston Public Schools, you may file a report to the Office of Equity. Upon receiving this report, the Office of Equity will conduct an assessment of the report and assist with next steps to help resolve the situation.

Reporters are encouraged to complete a written report using the form below, but are not required to. If you would prefer to discuss your concerns in person or over the phone, you may decline to complete this form by selecting option B below. A representative from the Office of Equity will then contact you to talk with you further, generally within two business days.

Information in your report, including the parties involved in the report and the investigation, will be kept confidential to the extent practicable. The Boston Public Schools does not tolerate any form of retaliation against individuals who report or participate in an investigation of an alleged violation of the district’s policies.
