Membership Cancellation Form
We're sorry to see you go, and we get it. Things change. Please use this form to cancel your membership. Until the club receives this formal notification, you will remain an official member of Budd Bay Rugby and are responsible for all membership fees. Once this form is received and accepted by the Club President, you will stop being charged membership fees, and refunded any charges that may have appeared after receipt of this form. And, please take a minute to tell us a little about your experience, and how we can do better in the future.
