This form is intended to be used to notify the Title IX team if a student or colleague has disclosed to you an incident involving sexual misconduct. This is an internal institutional document that alerts a Title IX Coordinator so that they can reach out to the individual and work to ensure that appropriate care and support is administered. The individual still has an option at that point to decide whether or not to pursue a formal complaint. In addition to this form, you can also encourage the student or colleague to utilize one of our confidential support services on campus: Campus Pastors Office or University Counseling Center.
