1. Acknowledgment and Assumption of Risk

I, voluntarily choose to participate in services provided by Trisha Hayden, with The Springs Wellness Center (hereinafter referred to as "the Company"), including but not limited to coaching, yoga, Reiki, somatic movement, retreats, and other healing modalities (hereinafter referred to as "Services").

I understand that participation in these Services may involve physical, emotional, and mental risks. These may include, but are not limited to, physical exertion, emotional vulnerability, and the processing of personal feelings. I acknowledge that I have voluntarily elected to participate and fully assume all risks associated with the Services.

By signing this waiver, I affirm the following:

1. I am in good physical health and do not suffer from any condition that would prevent or limit my safe participation in the Services.

2. I understand that the Services are not a substitute for medical care, diagnosis, or treatment, and I agree to consult my healthcare provider before beginning any exercise or wellness program, especially if I have any pre-existing conditions, injuries, or concerns.

3. I voluntarily assume the risk of injury, illness, or emotional discomfort that may arise during participation in the Services.

2. Waiver of Liability

I agree to release, indemnify, and hold harmless Trisha Hayden and The Springs Wellness Center LLC, its instructors, employees, contractors, and affiliates from any and all claims, demands, losses, damages, or legal actions that may arise from participation in the Services.

This includes, but is not limited to:

· Injuries or illnesses sustained during yoga, Reiki, somatic movement, or any healing modality provided by the Company.

· Emotional or psychological discomfort arising from coaching or personal development sessions.

· Injuries or discomfort related to movement, stretching, or physical activity.

I understand that:

· I am solely responsible for my own health and safety during participation in any of the Services.

· I will immediately notify the Company if I experience any discomfort, pain, or injury during the session and will discontinue the activity if necessary.

3. No Guarantee of Results

· I understand that Trisha Hayden and The Springs Wellness Center LLC and its instructors make no representations or guarantees regarding the outcome of the Services. Coaching, yoga, Reiki, Retreats, and somatic movement are personal development and wellness tools, and individual results may vary.

· I acknowledge that my personal growth and healing are influenced by multiple factors, including my own engagement and commitment to the process.

4. Confidentiality and Privacy

The Company respects the confidentiality of all client communications. I understand that all personal information shared during the Services will remain confidential unless I provide written consent to disclose it. Exceptions to this confidentiality may apply if required by law or in cases of imminent harm to myself or others.

5. Payment and Cancellation Policies

I acknowledge that all payments for Services are non-refundable unless otherwise specified by the Company. In the event I need to cancel or reschedule a session, I agree to adhere to the Company’s cancellation policies, which may require notice within a certain timeframe to avoid forfeiting the session fee (24 hour notice for services) I also understand that any down payment for retreats are nonrefundable if you choose to cancel. You will receive a 50% refund of the money you paid outside of the down payment if you need to cancel up to 4 months before retreat. Any cancellations within 90 days of a retreat will forfeit a refund and your spot may be filled by another attendee. You can, however, gift your spot to someone else should you need to cancel with approval of Trisha Hayden.

6. Agreement to Participate

By signing this waiver, I confirm that I have read, understood, and agree to the terms and conditions outlined in this document. I understand that I am voluntarily participating in the Services provided by Trisha Hayden and The Springs Wellness Center LLC at my own risk and will not hold the Company responsible for any injuries, discomfort, or outcomes arising from my participation.
