Elementary Reading Revolution Team Registration Form
This is the official registration form for the Reading Revolution Elementary Competition. You will register each team individually so that we can accurately track how many teams will compete. This year each team will consist of no more than 4 students, no alternates will be allowed. There will be a maximum of 2 teams per school allowed.

There will be a $25 team cost to participate in this event. Checks and PO’s can be made out to Tulare County Office of Education and mailed to Attn: Paula Terrill, 7000 W. Doe Avenue, Suite A, Visalia, CA 93291 prior to competition. Cancellations must be made in writing at least two weeks prior to competition for a refund.

A media release will be needed for each student as well. Media Releases can be found on the website at commoncore.tcoe.org and faxed to Paula at 559-651-0679.

All forms and payments must be received prior to competition day. This year’s competition will take place on May 11, 2018 at the Doe Avenue Complex.
