Please fill out this form only if you want to DENY release of your student's directory information and/or publishing permission of photos, videos and student work. Fill out the form for each child you wish to DENY release without consent. Directory information could include Name, Address, Telephone, Email, Place of Birth, Grade, Homeroom, Student ID, Activities and Sports, Weight and Height, and Awards. Parent/guardian information is also given to school-related publications. (PTO, Boosters, Wayland Public Schools Foundation)

WPS occasionally posts photos, videos or student work in the news, through emailed publications and on school websites and/or online websites such as teacher blogs.   We may post photos and videos from school events and field trips. We rarely use a student's name, and if used, it is first name only.
