3:16 Sports Flag Football Registration 2018
Reminder: Each child will need to attend one evaluation session. Sessions will be held Saturday, August 25 from 10am to 1pm and Monday, August 27th from 5pm to 7pm at FBCWC Community Campus weather permitting. Drop in at any time during the session. The evaluation will take about 10 to 15 minutes. As a reminder, the participation fee of $25 will be due at the evaluation session. If you have multiple children, please fill out separate registrations for each child.
At the session, a parent or guardian must sign an Authorization and Release of Liability, Participation and Safety Form, and a Consent to Medical Treatment Waiver.
