Family Success Center Online Pre-Assessment
The online pre-assessment is the first step of the application process for any of the Family Success Center's Temporary Financial Assistance programs. Please complete this form and a Family Success Center representative will reach out to you as soon as possible.
How many people in your household (including yourself)?
If you are seeking assistance with back rent, your household includes all of the people with whom you currently live. If you are seeking assistance with start-up costs, your household includes anyone who will be living in your new unit.
Do you have children under 21 in your custody?
Where are you currently staying?
In own apartment/house
Doubled up: Staying with a friend or family member
Domestic violence shelter
Somewhere not habitable (outside, car, hospital emergency room, etc.)
What is your housing crisis?
Eviction: I have a Court Summons with an upcoming court date (or have already been to court).
Eviction: I have a 14-day or 30-day Notice-to-Quit, but I have not yet received a court date.
Doubled up: I am staying with a friend or family member and must leave.
Utilities: I have a scheduled shut-off date for one or more of my utilities.
Foreclosure: I am the homeowner and have received foreclosure paperwork.
Health & safety: The Board of Health has determined that I can no longer live in the unit.
Domestic violence: I must leave where I am staying due to risk of my personal safety.
