家長可以親臨或郵寄方式遞交報名表。請集齊以下文件，如不齊備，恕未能處理。
Parents may submit application form in
person or by post. The following documents are required to be submitted
together with the Application Form, any application with outstanding document
will not be processed.
（一）兒童出生證明書副本
Photocopy of the child’s birth certificate
（二）回郵信封三個（請貼上$2.2 郵票及寫上地址）
Three stamped self ~addressed envelopes
(HK$2.2 stamp and mailing address)
（三）報名費$40（報名時繳交，報名費一經繳交，恕不發還。）
Enrollment Fee: HK$40 (to be collected together with the applicant form. The paid Enrollment Fee is not refundable.)
