NMT Campus Visitor Access Form

The New Mexico Tech visitor policy under COVID-19 restrictions is designed to be in compliance with the State of New Mexico Executive Orders. (https://www.governor.state.nm.us/about-the-governor/executive-orders/)

(a) Visitor Approval
Prior to arriving on campus, all visitors are required to fill out submit the following form to obtain permission to visit on campus. If the visitor is COVID-19 symptom free (see list of symptoms below) and has not been in contact with someone that has tested positive for more than 14 days, the visitor will be allowed on campus provided they comply with all State and campus mandates. If any of the above conditions are not met the visitor is required to self-isolate or self-quarantine for 14 days before re-applying for approval to visit campus.

If approved, on the day of the visit prior to coming onto the campus facility, the visitor must also agree to conduct a self-assessment to determine if they are experiencing any COVID-19 symptoms and also conduct a temperature check.

COVID-19 Symptoms include:
Fever over 100.0F without medication
Loss of smell or taste
Muscle ache
Sore throat
congestion/ runny nose
New or unusual headache
Nausea, diarrhea, vomiting, or loss of appetite

(b) Visit duration
The duration of any approved visit will be limited to the business activity specified on the attached form.
