SRSFL 2025 Volunteer and Hiking Guide Application:  
Our Summer Hiking Weekend is June 14th-16th at Truckee, CA.  Please read each question carefully and answer completely!  There is no charge for guides and volunteers unless you are staying at the hotel or joining us for meals.  Dinners are $30 each for those not staying at the Best Western.  Dinners are included if you staying at the Best Western.   The deadline to pay for the hotels is June 1st. Any refunds must be applied for by June 1st.   A Sacramento area day hike is also planned at the Cosumnes River Preserve walk on Saturday, May 10th.  If you are willing to guide at this day hike, make sure your application is in by May 1st.   The information and permissions in this application will include these events and all other possible summer hikes and activities held in 2025.

Consent for Emergency Treatment: The information requested on this form is for the sole use of medical professionals, in the unlikely event that you are seriously injured or become ill while attending a Sierra Regional Ski For Light event. 

To the hospital/clinic: Any and all costs associated with the transport, diagnosis, treatment, or any other care given to the named individual on this form is the sole responsibility of said individual.  The named individual is consenting to this by signing the form.  In the event that I should require any medical or surgical treatment and/or medication during  the course of  attendance at or participation in the Sierra Regional Ski For Light program, I authorize such physician or medical staff as Sierra Regional Ski For Light may appoint or designate to carry out the necessary treatment, or to take me to the emergency room of the nearest hospital, and I further authorize the hospital and its medical staff to provide the treatment deemed necessary by them for my well-being.

Guide/Volunteer Waiver of Liability-2025
I voluntarily agree to participate as a volunteer guide and/or other assigned volunteer responsibility in Sierra Regional Ski for Light Programs. This document sets forth the responsibilities and understandings of the volunteer and of the Sierra Regional Ski for Light (SRSFL) regarding volunteer’s participation in SRSFL programs.
1. I recognize that my service is of my own free will, without promise, expectation, or receipt of remuneration. The volunteer is not an employee or agent of the SRSFL for any purpose and the volunteer’s services are not controlled nor mandated by the SRSFL.
2. I understand and agree that it is possible that I may be injured or otherwise harmed during volunteer service due to accidents, acts of nature, my negligent or intentional acts, or the negligent or intentional acts of others; that while the SRSFL has taken some steps to reduce the chances of injuries or harm to a volunteer,  SRSFL has no control over most risks, and thus, cannot and does not guarantee nor take any responsibility for the safety a volunteer or the volunteer’s property while the volunteer is engaged in volunteer service. The volunteer must take full responsibility for himself or herself and assume the risk of harm or damage while serving by taking all necessary and reasonable precautions and acting in a manner that will help protect himself or herself and his or her property.
3. I agrees to waive and release SRSFL from any and all potential claims for injury, illness, damage, or death which I may have against the SRSFL that might arise out of my service and to hold the SRSFL harmless there from.  I agree and understands that injuries or losses to others, such as other volunteers or the person(s) being helped, may occur as a result of the my negligent or intentional acts during volunteer service and that to avoid such harm, I must exercise care and act responsibly in serving others.  If any injury or loss to another does occur due my intentional actions or due to the my negligent actions arising outside of the scope of the my activities, I must accept the liability for and repair, or make reparations for the harm done.
4. I must maintain his or her own primary medical insurance and his or her own automobile liability insurance to cover potential medical and other costs related to the volunteer service.  All costs for injury or loss above the coverage provided by my insurance are my personal responsibility. In projects where I agree to transport others by vehicle, the I will be required to provide proof of automobile insurance in order to participate as a driver. I am also encouraged to maintain property and life insurance coverage while serving as a volunteer.
5. Since volunteers are not SRSFL employees, the SRSFL does not provide worker’s compensation coverage for injuries or illnesses to the volunteer arising out of volunteer activities.

By my signature at the end of this application, I hereby state that I understand the risks, known and unknown, involved in participating in a SRSFL program and willingly and voluntarily accept these risks. By my signature, I hereby release from any claim, indemnify and hold harmless, surrendering any right to seek reimbursement from SRSFL and its board members, officers, volunteers and other agents for injury sustained and loss or liability incurred during my participation in the activities described above. By my signature, I warrant that I am not relying on any oral representations, statements or inducement apart from the statements made on this form.

Photo/Model Release: I hereby give Sierra Regional Ski For Light and its programs permission to use my photograph, and to publish the same without incurring any debts or liabilities to me of any kind.

Our Three Day Hiking Weekend is June 14th-16th.  Please answer the following questions about this event!
Please check the days you will be available to guide.  All 3 days are strongly encouraged.
Do you plan to stay at the Fairfield Inn in Rancho Cordova on Friday night, June 13th?  Also, do you wish double occupancy at a cost of $80.00.  This includes $10.00 for a pizza and salad dinner. For a single room add an additional $70. total for a single room $150 including pizza dinner.  If double occupancy indicates a roommate preference.
If you need lodging in Truckee, it will be at the Best Western and  your cost will be $250.00 for double occupancy.  If you wish a private room add $200.00.   Dinners are included.  The deadline to pay for the hotels is June 1st. Any refunds must be applied for by June 1st.

 SRSFL will provide dinners both evenings. If you are staying at the hotel the cost of your dinners is included.  If you are not staying at the hotel dinners are $30.00 per person per meal.  Please mark which dinners you plan to attend.
