Mask Up Dayton - Non-Profit + Business Mask Request Form
Mask Up Dayton is a project hosted by the Dayton Sewing Collaborative.  The purpose of the project is to make and donate masks to healthcare and human services organizations throughout the Dayton region.  To date over 9,000 masks have been distributed as result of the project.

The face masks we are making are a simple two-layer cotton design with around-the-ear elastic or ties/straps.

Before you complete and submit this form, please note the following:

- In an effort to minimize and manage the requests, a single point of contact for each organization or business must be identified; this person should complete the form below.

- Please complete all fields on the form. Failure to provide the required information will delay handling of request.

- We are unable to accept requests from individuals at this time.

- Masks will be available at no charge.
