2025 Project Prom Dress SA Application
ARJ Organization DBA Project Prom Dress San Antonio (PPDSA), is a 501(c)(3) non-profit organization in San Antonio, Texas whom receives donations of items and services that are provided to selected recipients and their families free of charge.  Items and service donations can include (but are not limited to) dresses, suits, dress shirts, slacks, shoes, accessories, & hair and/or makeup services from local stylists (Seniors Only, Prom Only, not a guarantee for hair and/or makeup). We also ask to take pictures when selecting your items and receive a picture from the night of your event. Along with promoting our organization, this insures us the item(s) are being worn and services aren’t abused.

Please answer all questions. Info will remain confidential. Teens will be seen by appointment only. We are a small team who all work full time jobs and will try our best to assist with unexpected needs. We will text and/or email (PLEASE CHECK SPAM/JUNK FOLDER) to confirm your selected appointment day. Under 18 must be accompanied by parent/guardian during appointment.

Photo Release:  For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby authorize ARJ Organization DBA Project Prom Dress San Antonio (‘PPDSA’) permission to use my likeness in a photograph in any and all of its publications, including but not limited to all PPDSA printed and digital publications. I understand and agree that any photograph using my likeness will become property of PPDSA and will not be returned. I acknowledge that since my participation with PPDSA is voluntary, I will receive no financial compensation. I hereby irrevocably authorize PPDSA to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing PPDSA’s programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless and release and forever discharge PPDSA from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
