Located along North Oakland Avenue on Milwaukee's Upper East Side, 3055 North Oakland is situated within one block of the University of Wisconsin-Milwaukee. Both Indoor Garage and Outdoor Off-Street on-site parking are available for rental. Laundry facilities are right on the premises; there are three washers and three dryers, all coin operated.

Service or Assistance Animal ("SAA") or Emotional Support Animal ("ESA")

A. A signed, dated, written and notarized statement, under penalty of perjury, from a licensed doctor or licensed therapist which is physically located in the State of Wisconsin, or from another State in which the applicant is originating, on the doctor’s or therapist’s official letterhead indicating the need for an SA or ESA.

B. Copy of the practicing physician’s or therapist’s current and valid License issued by the State or Wisconsin (this must come directly from the State of Wisconsin or other relevant State authority), or if meeting the Emanating state requirements, the relevant state, with a certificate or documentation from the relevant State authority that the physician or therapist is in good standing.

Misrepresentation(s) by the applicant to the landlord regarding this policy is subject to both civil and criminal pursuit of prosecution for such misrepresentation(s). Applicant must declare in advance to landlord that the tenant is in need of a SAA or ESA, unless the need is readily apparent, i.e. a Seeing-Eye (TM) dog is a readily apparent need. NOTE: Due to limitations of staffing and allergic reaction concerns, tenant will have to abide by Landlord's conditions of rental regarding Service or Assistance or Emotional Support Animal. Earnest Money Deposit Required
Applicant must submit a $100.00 earnest money with this form in order to demonstrate that applicant is serious about the apartment for which applicant is applying. The earnest money deposit is returned to the applicant if landlord doesn't approve the application; otherwise, it is applied towards the full security deposit upon approval of your application.

Each adult person who will occupy the apartment must complete a separate Apartment Application and each applicant must submit a separate Earnest Money Deposit, minors excepted in the company of an adult.

If landlord approves the application, applicant then must remit the full Security Deposit (Security Deposit less Earnest Money Deposit) in an amount equal to 1x the rental amount of the apartment when the applicant meets the income requirement below or provides a Cosigner.

If the applicant is unable to meet the income requirement or doesn't provide a Cosigner, the deposit required will be equal to 2x the rental amount of the apartment; it is not a rent payment.

Applicant shall submit the Earnest Money Deposit via PayPal (paypal.com) to the following e-mail address: waltoninn@sbcglobal.net. NOTICE: This payment must come from applicant's bank account, rather than applicant's debit or credit card, since PayPal will charge a fee when applicant uses a debit or credit card. Day Company will not be responsible for any fees (from PayPal) deducted from the payment as a result of the applicant using a debit or credit card to fund the payment or any other payment from tenant to landlord, including rent, and thus those fees must be paid by the applicant.

Once the applicant is approved, upon signing a Residential Rental Agreement, tenant will be responsible for remitting the balance of the deposit (Security Deposit less Earnest Money Deposit) and the First Month's Rent. For example, for a 1 Bedroom rental:

Renter's Insurance Required
Each tenant is required to carry his or her own renter's insurance, and Day Company must be named as a party to be notified of any changes to the policy, i.e. cancellation, non-payment of premium, claim of loss, etc. Applicant will be required, upon approval of the application, to submit proof of insurance to Day Company in the form of a Certificate of Insurance to be sent to Day Company via email to: daycompany@gmail.com or via U.S. Mail to: Day Company, Post Office Box 170706, Milwaukee, WI 53217-8061. Note: Renter's Insurance is usually available for a cost of between approximately $10.00 to $20.00 per month, depending on the coverage, so the applicant is encouraged to take this amount into account when planning monthly expenses of renting.

Each applicant must meet this requirement, since the application is joint and several; this means that each applicant is fully responsible for the rental payments, so if one tenant doesn't pay, each person is responsible for the full payment until paid in full. Applicant understands this is an application for an apartment rental and has read all pages, which are the Terms and Conditions of application. Landlord may undertake at any time a credit and background investigation of person signing below, including any history and character. Information may be exchanged now and in the future between all relevant parties deemed so by Day Company. All information on this form is true, complete and correct to the best of applicant’s belief and knowledge and that if any information provided is found to be untrue, incomplete or misleading, this will be cause for landlord to reject the application or terminate the tenancy, even after occupancy of the apartment. A 60-day written notice is required at the end of the lease term or at any time thereafter. Landlord makes no promises of any aesthetic improvement to the apartment. Applicant must maintain personal property, liability, accident insurance at all times; landlord is not responsible for damage, injury or loss, not due to landlord’s negligence; presumption is landlord is not negligent. All rental payments are due in advance on or before the 1st of the month; check or money order payable to Day Company. No cash payments are accepted. Applicant certifies Applicant was shown the apartment indicated above, or any subsequent to which Applicant is shown, furnished for inspection a copy of the Residential Rental Agreement, Nonstandard Rental Provisions and Resident Handbook, the “Terms and Conditions”, and Applicant agreed to them before Applicant gave a deposit or submitted an application. If Applicant has a Cosigner, Applicant certifies that Applicant has read the Terms and Conditions on behalf of such Cosigner.
