Sylvan Union School District Child Care Program Enrollment
This form will be used to complete enrollment to the Child Care Program for Sylvan Union School District. Enrollment is open for the following students for afterschool care: Students attending Freedom, Mary Ann Sanders, Orchard, Stockard Coffee, Crossroads, Ustach and Dan Savage.
Enrollment is open for ALL SUSD STUDENTS for before school care.
Enrollment is open for ALL TK AM students. TK AM Students are bussed to our Main Site at 1037 Floyd ave for afterschool care.
Please refer to our Child Care link on the District Website for hours of operation, and to review the online parent handbook of policies and procedures.
You will need to fill out one form per child registering.
Afterschool care for Sherwood, Standiford, Woodrow, Sylvan Steam, CFB and Somerset will require you to register with The  Boys and Girls Club @  bgcstanislaus.org
