Admission Requirements

Freshmen Applicants
  1. PSA Birth Certificate
  2. Original High School Report Card w/ LRN
  3. Photocopy of Junior High School Completion Certificate
  4. Original Certificate of Good Moral Character
  5. ESC Certificate letter from Junior High School Principal
  6. QVR Certificate (Qualified Voucher Applicants Only)
  7. Two (2) copies each of "1x1" and "2x2" pictures ( white background)
Transferees
  1. Original copy of Form 137 from previous school
  2. Original Certificate or Good Moral Character coming from the School Head of the previous school
  3. Photocopy of Birth Certificate (PSA Authenticated)(bring upon enrollment)
  4. Two (2) copies each of "1x1" and "2x2" pictures ( white background)

  • For documents that are to follow: Required documents/requirements can be submitted within 1 week after class start.
  • To secure your enrollment slot, the applicant may pay the Reservation fee / Admission fee of Php 500.00 (SHS Students) through the following online payment option or you may transact directly at the school's Accounting office during office hours.
