By completing this form, I agree to the following method of payment to Denver Academy of Ballet for the 2021/2022 season.

If I wish to withdraw from any class I must give a 2 week notice prior to the start of the following Quarter.

Only tuition payments and registration fees will be charged automatically to my account. All other payments (i.e. performance fees, merchandise, Company, competition fees etc.) must be paid for with separate authorization.

For auto debit a current credit card must remain on file. Checks can be submitted to the new tuition box in the lobby.

I authorize Denver Academy of Ballet to auto debit/charge fees (full quarterly tuition) from my account each Quarter for tuition one week prior to quarter start date.

I authorize Denver Academy of Ballet to auto debit/charge fees (1/2 quarterly tuition) from my account every 5 weeks. First charge will be one week prior to quarter start date and second charge will be 5 weeks later.

I will be submitting a check or cash for the full quarterly tuition amount on or before the start of the quarter. One week after the quarter begins a late fee of $30 will be charged to my account.
