The purpose of this form is to capture and report events that occur during Cromwell Chill club sponsored events e.g. practices, games, tournaments etc.
Reportable events are any action that breaks either Cromwell Chill's or the the other venues Rules and regulations.
Reportable events may be submitted by anyone that witnesses the event first hand, including Coaches, Players, Parents, Family, Officials or spectators in general.
Reportable events aren't limited to actions that occur only in cromwell but can be any event which involves Cromwell Chill players or personnel.
Upon completion , a notification of this submittal will be sent to the Club President who may contact you for further details.
