Note: Not all absences will be approved, you will receive an email response ONLY if your absence is UN-excused within 72 hours of submission.

**Please submit all absences 2 weeks prior to date requested.
**Any athlete with three unexcused absences may forfeit their role on the team.
**Any athlete that misses a practice the 2 weeks before a competition may be pulled from the routine for that weekend. 
**For any unexpected absences that may come up please email the office at info@naturalvenomallstars.com, this form should be used for planned absences.
