IMPORTANT PLEASE ONLY FILL OUT ONE REQUEST PER FAMILY. WE WILL CONTINUE TO PROVIDE MEALS FOR YOUR CHILDREN UNTIL YOU TELL US TO STOP. IF YOU DID NOT ADD ALL OF YOUR CHILDREN ON THE FRIST FORM PLEASE SUBMIT A SECOND FORM FOR ADDITIONAL CHILDREN AND DO NOT INCLUDE THE ORIGINAL CHILDREN IN THAT FORM. In an effort to support the families of the Clyde-Green Springs Schools we will be offering grab-and-go breakfast and lunch meals for students during the school closure. Meals will have all components as required by the USDA. Meals will be a combination of perishable, non-perishable and shelf-stable items. All children 18 and under, not just students who are eligible for the Free and Reduced Meal Program, are able to receive these meals. Please complete the information below if your family is interested. The NEW Pick up times will be on Monday and Thursday from 11:00-12:30. On Monday you will receive 3 breakfast and 3 lunch meals, and on Thursday you will receive 2 breakfast and 2 lunch meals.
