To withdraw from lessons, complete this form by the 15th of your final month. A minimum of 15 days notice is required to withdraw from lessons. Requests received after the 15th of the month will apply to the following month. Once this form is received, a CMS staff member will contact you at the email address provided to confirm the cancellation of your lessons and billing plan.
