CMS Lesson Withdrawal Request Form

To withdraw from lessons, complete this form by the 15th of your final month. A minimum of 15 days notice is required to withdraw from lessons. Requests received after the 15th of the month will apply to the following month. Once this form is received, a CMS staff member will contact you at the email address provided to confirm the cancellation of your lessons and billing plan. Upon submission of this form, CMS will start to process your withdrawal. You will still be expected to attend/pay for any lessons that fall within the 15-day notice period.
