This is the feedback and appeal form for Shift Booking PILOT system that is running from 02 November 2020 to 31 January 2021.

1) There will be no replies to questions submitted via this form.
2) Only deliver-partners who received 3 strikes and have their account banned will have their submission processed. All other submissions/appeals for strikes here will NOT be processed.
