If you are filling out this form you are agreeing to pay for your player via Venmo (online @LavacaYBSA), Cash (in Person), or Check (in Person). Stay at Home Teeball cost is $30 per player, and Travel Teeball, Baseball, and Softball cost is $40 per player.

Coaches may ask for $10 if they elect to do a jersey instead of a dri-fit t-shirt. 

In Person sign ups and payment drop-off will be at East Sebastian County Ballpark, 707 Featherhill Road, Lavaca, AR 72941.

There will be NO REFUNDS once my child has been accepted on a team and the first practice is held. LYBSA will not be liable for any injuries or losses whatsoever to the player during practices, games, or any activities that are related to the LYBSA during the 2025 season. Your child will not be covered by any insurance from the LYBSA and the LYBSA is hear-by released from any injuries or losses. As a parent or guardian, I agree to abide by the code of conduct for myself, my guests, and my child.
