South Prairie Parent Survey on School Restart
*Please fill out one survey for each child.
During this time of so much uncertainty, we have been waiting to ask for your ideas and concerns and opinions until we received more information and guidance from the state. As we make decisions toward beginning in the fall, we are asking your opinion in regards to what our restart may look like.
Our priorities are to ensure the health and safety of our students and staff and provide the best learning opportunities possible for our students.
Please know that your ideas, concerns, and opinions are deeply valued. We know that there is not a simple or perfect solution, and we are doing our very best to meet the needs of the students, staff, and community as we move forward with developing a plan.
I am a parent/guardian of a South Prairie
Junior High Student
High School Student
Has this student been diagnosed, by a health professional, as at-risk due to COVID-19?
If school were to start today in a face-to-face setting, would you send your student?
If you answered "Maybe," on the previous question, please explain why:
If today was the first day of school, I would prefer
Full-time in-person learning at school.
Combination of in-person and online learning to minimize the number of students at school
Virtual academy online (100% distance learning)
I will unenroll and homeschool my child (I understand that I would be responsible for providing curriculum and progress monitoring for my child).
What are your feelings about students wearing masks during the school day?
Students should be required to wear masks at all times.
Students should be required to wear masks when social distancing is not possible.
Students should be encouraged, but not required to wear masks when social distancing is not possible.
Students should not have to wear masks unless they choose to do so.
Transportation is an important factor when considering re-opening. The following considerations would be part of our transportation plan as social distancing will not be possible on district transportation:
-Anyone who is sick, displays COVID-19 related symptoms, or has had contact with a person who has COVID-19 should not use district transportation.
-Students should wash their hands (or use hand sanitizer) prior to using district transportation.
-Hand sanitizer will be available.
-It is recommended students use facial coverings as it is impossible to ensure social distancing on district transportation.
For district transportation, pick the statement that best describes your plan for the fall given the statement above (PLEASE MARK ONLY ONE):
My child WILL use district transportation
My child will NOT use district transportation
We are uncertain at this time
