As we prepare for the first day of school on August 17, 2020, we need to know how to place each student. Please complete one form per child for the option you choose.

I understand and agree to all of the following requirements for enrolling my child in distance learning: (ALL BOXES MUST BE CHECKED TO COMPLETE ENROLLMENT)
Daily attendance in the virtual classes is required.
Daily live interaction with the teacher and classmates is required.
Devices and hotspots will be provided for families in need.
Quality, challenging content aligned to grade level standards, equivalent to in-person instruction will be provided.
District-adopted instructional materials will be used and will be sent home.
Assessments will be given.
Grades will be reported each quarter on the ACUSD report card.
Teachers will continue to provide intervention, English Learner support, and special education services for identified students.
Counselors and other school support staff are available to assist my child if needed.
