This purpose of this questionnaire is to provide accurate information about your photography needs so that we bring ample equipment (for example, lighting) for the location. Your satisfaction & our ability to provide you with beautiful images is very important to us. Communication is the key to achieving the images you're hoping for, but by the same token, don't fret over the answers too heavily. This is our first point of contact. Just share what you know to be true. (If any changes occur, just relay those to us in e-mail at least 72 hours prior to the event & we will do our best to accomodate those within the means we've got available.)

What is the physical address of the location you need photography services?
Will there be any location changes (address changes) during the event? If so, please explain & provide the address of the new location.
What are the hours of coverage you need? (example: “from 7 p.m. to 11 p.m." or simply state "standard portrait session.")
Do you need the photographer to capture outdoor or indoor images?
Does the event need special equipment? (example: backdrop/stand/lighting setup) Will there be a wall in which we can bounce lighting off of? Will this event have dimmed lighting conditions?
Are there any specific photographs we should aim to capture? (example: “we definitely want a photo of my brothers and I with grandma”)
Do you expect any interesting challenges we may need to know of in advance? (example: uneven terrain/fall risk, excessive movement/boat on water, disk jockey with strobe/colored lighting, no parking near event, etc.)
