The following request form is for the 2021-2022 school year. The coordinator of the program you selected will contact all parents/guardians after review of the request form to provide further information.

*IMPORTANT NEXT STEPS NEEDED IN ADDITION TO THIS REQUEST FORM:

1. All parents/guardians must enroll ALL of their children in one account even if the students attend different schools in the district through our Online Registration (OLR) system. For more information on OLR, please visit: http://it.sweetwaterschools.org/infinite-campus-parent-portal-guide/

2. If you are requesting a program at a school other than your school of residence (according to your home address) you will also need to apply for an Intradistrict Transfer during our Open Enrollment period (April 5 - May 7, 2021). For more information on Open Enrollment, please go to http://studentservices.sweetwaterschools.org

El siguiente formulario es para el ciclo escolar 2021-2022. El/la cordinador(a) del programa que seleccionó contactará a todos los padres de familia/tutores con más información después de revisar su formulario.

*ADEMAS DE ESTA FORMULARIO, SIGUIENTES PASOS DE IMPORTANCIA REQUERIDOS :

1. Todo padre de familia/tutor deberá inscribir a todos sus hijos bajo una sola cuenta aunque sus hijos asistan a diferentes escuelas en el distrito por medio del Registro en Linea - Online Registration (OLR). 2. Si está solicitando un programa en una escuela diferente a la que le pertenece por residencia/domicilio, deberá también aplicar para un cambio de escuela - Intradistrict Transfer durante el periodo del 5 de abril - 7 de mayo, 2021.
