Use this form to report any student concerns that occurred on school property, at a school-sponsored activity or event off school property, on a school bus, or on social media or through text message during the current school year.

Confidentiality: You do not have to include your name on this form. However, a staff person may want to ask more questions, so including your name will help with follow up on this report. If you do include your name, an effort will be made to keep your name confidential. However, depending on the situation, some information may need to be shared with others as part of the investigation of this report. Please note that discipline decisions cannot be made solely on anonymous reports.
