Form to Request Prior Approval of CEUs (For a FUTURE lane change) - PT, OT, AUDIOLOGY, BCBA, MHP, SLP, LSN ONLY!!
Instructions:

1. Complete this google form.  Submit ONCE per course for prior approval (you may want to save a PDF of the form before submitting it, for your records).  
2. Review the form to make sure there is adequate information and time for review and approval by all parties prior to the start date of your course(s) (typically, three (3) weeks minimum).
3. Once the form is submitted, it will automatically be reviewed by:
- Your supervisor (Assistant Director/Principal)
- The Executive Director
- The Superintendent
4. Once the form has been fully approved (ideally within a week of your submission), a scan of this form will be emailed to you at your ISD 917 account indicating the approval or non-approval.  You will need to submit the scan/copy with your lane change request, after the credits/units are earned.

NOTE:  All credits or continuing education credits (CEUs) submitted for approval must be future credits, not yet earned. Final course grade must be "B" or higher - not "no grade" etc. (see contract). Credits for "+10, +20, +30" must be earned after degree (BA or MA) date awarded.
