THE FUSE PROJECT - Sign Up Form
Thank you for your interest in wanting to sign up a young person to The Fuse Project!
If you are a parent/carer or professional, please complete this form.
Please make sure you can say 'yes!' to all of the eligibility check list below before completing the sign up form.
We want to make sure the project is suitable for everyone involved. This means, if any of the following limitations are evident, please get in touch to discuss suitability before submitting a sign up form.
It doesn't mean we'll automatically say no, but a conversation and needs assessment before sending a sign up form may be required to make sure we can support the young person as appropriately as possible.
1. Their behaviour could present a risk to themselves and/or the mentor
2. They have moderate to severe learning difficulties and/or disabilities
3. They have moderate to severe mental health difficulties
4. They have significant challenges requiring support not currently in place
5. They are unable or unwilling to meet in public spaces and/or via digital platforms
Please note, submitting a sign up form does not automatically accept the young person for the project. Once the form has been submitted, we'll review it and be in touch. WIRED reserves the right to refuse referrals which are not appropriate.

Eligibility check list
Please tick each box to confirm the young person is eligible at this stage for The Fuse Project.
The young person is aged between 11–18 years old and lives in Dorset
They can build relationships mainly through conversation
They want to improve their wellbeing and mental health
They agree to take part and meet with their mentor for six sessions
They want to be involved in The Fuse Project as described in the information pack
In-person or online mentoring?
Please confirm the version of The Fuse Project the young person wishes to complete. Please note, if the young person lives in the Dorset Council area, they are only eligible for the online version. Young people in BCP are eligible for in-person and/or online.
