PLEASE NOTE THAT COMPLETING THIS FORM DOES NOT GUARANTEE YOU A SPOT ON THE TRIP. Chaperones will be selected and notified by a confirmation email prior to December 23, 2016. Since consistency is so important to a smooth trip experience, priority will be given to chaperones who can attend for the entire week. All chaperones will be required to complete paperwork for a criminal background check at a cost to the chaperone of $23. The background check application must be notarized, and chaperones will be responsible for paying the notary's fee (typically about $2). Chaperones who do not ride the bus must pay their own transportation costs including tolls. Unless they are employees of BCPS, all chaperones must also complete the free 2016-2017 BCPS volunteer training. (If you did this at any BCPS school or at the CMS Back to School Night this year, you will not need to do it again, and we will hold one additional winter training session for all untrained NorthBay chaperones.) Chaperones receive free lodging and meals for the week at NorthBay and only pay for their children's field trip cost. If a chaperone's availability changes from "all week" to a "partial week" prior to the trip, we reserve the right to replace the chaperone with someone from our wait list who can attend for the full week.
Chaperones will sleep in a student cabin with their own child, provided the child and chaperone are the same gender. A chaperone and a child who are not the same gender will not be assigned to the same cabin under any circumstances. Chaperones are generally with students from 7:00 a.m. - 9 a.m. each day of the trip and from 4:00 p.m. - 7:00 a.m. Chaperones may choose to accompany students to classes and activities between 9 a.m. and 4:00 p.m. but are not required to do so. Many chaperones use this time to catch up on their sleep or to check in with family and/or work.
