Fill out this form after EACH GAME. 0=Poor, 1=Not so good, 2=Good, 3=Very Good, 4=Excellent. AN AVERAGE, GOOD GAME WOULD GET A 2 IN ALL CATEGORIES. Teams should only receive a 4 or 0 if they spirit was better than any game you've ever played in, or if there was zero understanding from the other team about rules of the game.
