Please complete one form per injury, incident or learning event
Injury - when someone has been harmed from a single or multiple occurrence,
Incident - when there is not an injury but damage occurs (including reputational damage): it includes complaints,
Learning Event - when there is no harm or damage but it nearly happened or was likely to happen.
Please note that all completed forms go to a folder in the SOA Board Google Drive which has limited and password protected access. Personal information in the reports is treated as confidential. This information will be forwarded to British Orienteering if the injury or incident meets the Insurance reporting requirements but otherwise will not be forwarded. Summary anonymised information to aid learning will be shared from time to time on the SOA website.
