Before entering the facility or any class, each participant/dancer will need to complete a quick health screening checklist. A parent or guardian will need to complete the form for a participant/dancer who is not of the age of majority. If the participant/dancer has any of the following signs or symptoms, please keep him/her at home. This is to ensure the safety for all participants/dancers at the academy.
