Hello everyone.

On behalf of the Mental Health Committee at United Parish, we invite you to fill out this anonymous survey asking how we can improve in creating a safe space for mental health challenges, based on your personal experiences. We greatly appreciate the responses we received leading up to our mental heath service on Sunday May 12, and welcome more input as we plan our next steps in moving forward with this initiative. All responses are completely anonymous.
