We are partnering with Cal Volunteers as well for our volunteer events so please read the boxes below to find out about the amazing opportunities with the state to help your local community as well.

I, _________________________ (Participant, Parent, Guardian), hereby consent to and authorize myself or _____________, (the Minor) to act as a participant for Food Exploration & Discovery (Food ED) Compostpalooza (hereinafter event/activities). I fully understand that my/my child’s/children’s participation in event/activities exposes me/my child/children to the risk of personal injury, death, or property loss or damage. I hereby acknowledge that I/my child/children am/are voluntarily participating in this event/class and agree to assume any such risks. I hereby waive, release, discharge and agree not to sue the Food ED, Monrovia Community Garden, Mountainside Communion, the City of Monrovia, and their respective officers, employees, partners and agents (collectively the “Indemnitees”) for any injury, death, or damage to or loss of personal property arising out of, or in connection with, my/my child’s/children’s participation in the event/activities from whatever cause, including the active or passive negligence of the Indemnitees or any other participants in the event/activities. The parties to this AGREEMENT understand that this document is not intended to release any party from any act or omission of “gross negligence,” as that term is used in applicable case law and/or statutory provision. In consideration for being permitted to participate in the event/activities, I hereby agree, for myself, my heirs, administrators, executors and assigns, that I shall indemnify, defend, and hold harmless the Indemnitees from any and all claims, demands, actions or suits arising out of or in connection with my/my child’s/children’s participation in the event/class.
I HAVE CAREFULLY READ THIS WAIVER, RELEASE, INDEMNITY, HOLD HARMLESS AND AGREEMENT NOT TO SUE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT IT IS A FULL RELEASE OF ALL LIABILITY FOR MYSELF AND, IF APPLICABLE, FOR THE MINOR PARTICIPANT(S) LISTED BELOW, AND KNOWINGLY SIGN IT ON MY OWN FREEWILL.

By signing and approving this waiver, you agree for your name and email address to be shared with the California Climate Action Corps (CCAC), a program of California Volunteers, Office of the Governor.  You will be part of the California Climate Action Corps and receive exciting volunteer opportunities and updates of the work being done statewide and how you can affect climate change, including disaster preparedness and response, in your community.
