1. The parents or guardians submitting this application are those having legal custody over the child.
2. Every precaution is taken for the safety and good health of our youth attending youth ministry events, but in the event of accident or sickness, the Parish Staff and volunteers at St. Mary's Church are hereby released from any and all liability for any harm arising to our child while attending youth ministry events. Each youth must be covered by Provincial Health Insurance or equivalent medical insurance.
3. In the event of illness, parents/guardian will be notified immediately.
4. To my knowledge my son/daughter is in good health, and is capable physically and emotionally of participating in these youth ministry events.
5. While masking is not required, either decision is respected. Good hygiene and respect for personal space will be encouraged.
6.Please do not bring your child if they are sick.
8. I have read this application and I accept the conditions of enrolment.
As a parent or legal guardian, I give my son' daughter permission to participate in Youth Ministry at St. Mary's Church. At times, this may include transportation in a motor vehicle, in which case, youth travelling will be travelling with a fully licensed adult volunteer or staff member, including but not limited to the youth minister, music director, staff or Pastor. This agreement does not permit youth to travel in vehicles with other teens with a G1/G2 status. I am aware all volunteers and employees have met the requirements as set out in the Archdioceses's policy for "Strengthening the Caring Community" (https://www. archtoronto.org/about-us-departments.../volunteer-screening). Participation in these activities involves elements of risk such as injuries from accidental falls, or bumps and bruises. These accidents can occur due to the nature of the activity and without the fault of the participant, leaders or the church. I assume the risks associated and I understand St. Mary's Parish does not provide any accidental death, disability, dismemberment or medical expenses insurance for children participating in these activities. I also permit the usage of the child's picture on social media for the purposes of sharing and promoting youth ministry events at St. Mary's Parish. This may include, but not limited to, Facebook, Instagram, Twitter and the Parish website.
