Family Life Center / Gym - Gymnasium, dining area, full kitchen, food warming tower.

Fellowship Hall - Seats 45, kitchen, TV.

Small A/V Room - Seats 25-45, basic A/V

Note: Staffed A/V required for Worship Center Usage. Training is provided for other areas.  A/V Technician: $50-100

Note: If the wedding coordinator is provided from the church a fee will be required (TBD).

Do you require an A/V Technician (A/V Technician: $50-100 required for Worship Center Usage. Training available for other locations with no fee on the day of the walk-through.)

I agree, I affirm that:

  1. I will leave the facility clean and in the condition I found it.

  2. All trash will be removed from the premises and placed in the dumpster located outside in the church parking lot (bags must be tied).

  3. All furnishings will be returned to their original location. Lights and A/C will be turned off.

  4. I will only access the facility during the approved time.

  5. If using A/V equipment, I understand some systems require trained church personnel.

  6. I am responsible for any damage that occurs during my event.

  7. I will ensure all attendees behave respectfully on church property.

  8. If I need to cancel, I will notify the church office no less than 72 hours in advance.

  9. I understand the church is not liable for accidents, injury, or loss during the event.

  10. Approval of this application does not grant use to others or future events.
