This enrollment portal and process contains important guidelines and policy information. Each section contained in this portal requires your signature as well as information only you can supply. It is very important that you complete each section as accurately and completely as possible. It is also important for you to understand that each of these forms are required by the Ada City School District or by state or federal law and are provided to you primarily for the safety and well being of your child.
Upon completion of the entire process, a PDF copy of your completed enrollment forms will be emailed directly to the correspondence email you will enter at the end of the portal process. In addition to a copy of your forms, you will receive valuable information regarding enrollment confirmation, orientation & meet-and-greet dates, residency verification requirements, immunization requirements, as well as other important information regarding the beginning of the 2017-2018 school year. You will NOT need to bring a copy of your forms to enrollment confirmation. Those forms will be electronically sent to the Ada School Administrative Offices.
