This form can be used for any and all safety concerns (inappropriate dialog, reckless behavior, harassment, abuse, sexual assault), and anything else you think the YogaSlackers should know about. Please note that responses go to the entire leadership board, so this form can even be used to report a member of the board, if necessary.
